You’re still eligible for care even if you’re paid by check or cash.
If you’ve been injured on the job, your employer’s workers’ compensation insurance is legally required to cover the medical services for your accepted injuries. Rest assured, at Woodstock Chiropractic, we handle the billing process directly with the workers’ compensation insurance company. You won’t need to worry about sending bills to your employer’s insurance company – we take care of that for you.
Opening a claim is a crucial step in receiving the compensation and care you deserve. When you’re injured, immediately notify your boss, manager, or supervisor about the incident. They will then fill out Form 801 (for Oregon injuries) and send it to their insurance company to open your injury claim. If, for some reason, your employer cannot provide this form, we can assist you in completing it.
During your first visit to Woodstock Chiropractic, you’ll also fill out Form 827 (for Oregon injuries). This form requires you to provide details about your injury, and our expert doctors will add your diagnosis information. We’ll then sign the form and send it to your employer’s insurance company, further facilitating the opening of your claim. Rest assured, we have these forms available in both English and Spanish.
